HIPAA Notice of Privacy Practices

This Notice applies to the protected health information ("PHI") created or received by Si Se Pierde LLC and its affiliated licensed providers in connection with your care through sisepierde30.com.

- Request corrections/amendments to your health information.
- Request an accounting of certain disclosures.
- Request restrictions on certain uses and disclosures.
- Request confidential communications by alternative means or location.
- Receive a paper copy of this Notice upon request.
- Be notified in the event of a breach of unsecured PHI.
Our responsibilities
We are required by law to maintain the privacy and security of your PHI, to provide you this Notice of our legal duties and privacy practices, to notify you following a breach of unsecured PHI, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have, as well as any information we receive in the future.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
